Medicare Part B LIDOS

Starting October 4, 2004, all outpatient and inpatient Part B claims will required a line item date of service for each revenue code.

The reason for this requirement is that the CMS's system can't process a range of service dates for each revenue code on outpatient claims. (Although the HIPAA implementation guide allows this to occur)
